Phyllis R. Fulk, age 79, of Oak Hill, Ohio passed away Wednesday, December 7, 2016 at Holzer Medical Center in Jackson, Ohio. She was born in Oak Hill, Ohio, November 18, 1937, to the late Ora and Martha (Radabaugh) Snyder. She was a member of the Porter Freewill Baptist Church of Sciotoville, Ohio.

In addition to her parents, Phyllis is preceded in death by her husband, Ted E. Fulk; six sisters, Margaret French, Fern Adkins, Virgie Milhoan, Carol Hilderbrant, Anna Jean Smith and Donna Snyder.

Phyllis is survived by her children, Dianne Oliver, of Columbus, Ohio, Dennis (Sue) Fulk, Deborah (Larry) Herald, and Denise (John) Rakes, all of Oak Hill, Ohio; eight grandchildren, fourteen great-grandchildren and several nieces, nephews, relatives and friends.

Funeral services will be held 11:00 AM Monday, December 12, 2016 at the Lewis & Gillum Funeral Home of Oak Hill with Pastor Mark Price officiating. Burial will follow in Horeb Cemetery.
