Dale E. Provens, age 76, of Columbus, passed away Sunday, June 24, 2018 at Grant Medical Center in Columbus.

A public memorial service will be held Saturday, June 30th, 2018 at 6:30 PM at the Kingdom Hall of Jehovah's Witnesses, 7180 Refugee Rd., Canal Winchester, OH 43110.

Private graveside services were held at the Fairmount Cemetery in Jackson under the direction of the Lewis & Gillum Funeral Home of Jackson.
