Carol Ann (Shriver) Fairchild RN, CASC, BMC Administrative Director of Surgical Services, age 58, of Dalton, Massachusetts, formerly of Jackson, Ohio passed away Monday, November 14, 2016 at her home in Massachusetts. She was born November 30, 1957 in Oak Hill, Ohio to the late John William and Cora Lee (Cook) Shriver.

Carol came to Berkshire Medical Center in 2010 with an extensive background in the building of ambulatory surgery centers, and was instrumental in the development and operations of the new Crane Center for Ambulatory Surgery. At BMC, Carol had managed ambulatory surgery and endoscopy and was subsequently appointed to Director of Surgical Services. She had previously served as Vice President of Operations for Ambulatory Surgery Centers of America, where she helped to develop and manage centers in Ohio, Pennsylvania, Michigan and Massachusetts. Carol also previously served as Director of Ambulatory Surgery for Holzer Clinic in Ohio and was the Clinical Manager for the Medical-Surgical Unit at Holzer Medical Center.

She is survived by her two sons, Nathan Andrew and Adam Lee (Seohye) Fairchild; granddaughter, Clarice Fairchild; brother, John Shriver and her two nephews, John W. and Austin H. Shriver, as well as numerous other relatives and friends.

In addition to her parents, she is preceded in death by her husband, Clarence Fairchild.

Funeral services will be held 11:00 AM Wednesday, November 23, 2016 at the Lewis & Gillum Funeral Home of Oak Hill. Burial will follow at Hill Cemetery.
